Lifetime to Opportunities 

Our Lifetime to Opportunities programs offer community residential services and crisis respite services to people with cognitive, mental health and/or physical challenges.

Opportunity Matters, Inc. currently owns and operates 18 community residential services homes located in the cities of St. Cloud, Sauk Rapids, Sartell and St. Joseph. The individuals served enjoy a safe, welcoming environment in which to live and experience their  community. There is a 24-hour staffing who help support the individuals with gaining or maintaining their independent skills and work toward their personal goals. A person center approach is utilized to develop the support that each individual is wanting in their life.

These programs serve individuals on the DD/CADI/BI/CAC Waivers or private pay. Our sites are licensed by the Minnesota Department of Human Services under 245D Home and Community Based Services. 

Community Residential Services

Crisis Respite Services

Opportunity Matters' crisis respite services provides immediate intervention for individuals experiencing a medical, family, or behavioral crisis.

Programs such as behavior management or a medical plan may be provided that can be tailored to the individuals immediate needs, along with assessment of needs, recommendations and referrals to appropriate agencies. Wrap around services may also be provided, so assessment and tools recommended can be put in place when the individual leaves crisis, to ensure success in their previous or new placement. Crisis services offer individual, secure units located within three of our foster homes and generally consist of short term stays between 30-180 days. Transportation can be provided if necessary.
