Our Residential Sites
Foster 1 is located in West St. Cloud and is home for 2 individuals. It is a home for individuals that need minimal support and are highly independent. Staff provide support with improving or maintaining independence skills, helping to access community resources, shopping, preparing meals, housekeeping, money management, social skills, medication monitoring and medical support.
Foster 2 is located in West St. Cloud and is home to four individuals. They enjoy music, sporting events, or anything that gets them out and into the community. Staff provides support with increasing independence skills, social skills, behavior management, support with medical needs, medication monitoring, preparing meals, housekeeping, money management, personal cares, and shopping.
Foster 3 is located in West St. Cloud and is home to four individuals. They enjoy sports, music, movies, trying out new local restaurants, and getting out in the community. Staff provide support with improving and maintaining independence skills, busing and accessing community transportation, behavior management, social skills, medication monitoring,meal prep, shopping, housekeeping, money management, and accessing community resources and activities.
Foster 4 is located in East St. Cloud and is home to four individuals. They enjoy spending time together, working on puzzles, arts and crafts, music, visitors and in-home activities. Staff provides support with medical needs, personal cares, medication monitoring, sensory needs, social skills, behavior management, preparing meals, shopping, housekeeping, money management, leisure and recreation at home and in the community.
Foster 5 is located in Sauk Rapids and is home to four individuals. They enjoy arts and crafts, movies, sports, and community outings. Staff provides support with functional skills, behavior management, sensory needs, social integration, preparing meals, housekeeping, money management skills, medication administration monitoring, shopping, and community resources.
Foster 7 is located in Sauk Rapids and is home to three individuals. They enjoy arts and crafts, movies, music entertainment, socializing, sports, and community outings. Staff provides support with medical needs, personal cares, medication administration, behavior management, social skills, preparing meals, housekeeping, shopping, and money management.
Foster 8 is located in St. Joseph and is home to four individuals. They enjoy art, video games, music, movies, and staying very active in the community. Staff provides support with life skills to increase independence, community integration, behavior management, money management and employment skills.
Foster 9 is located in West St. Cloud and is home to four individuals. They enjoy staying active by frequently going on community outings to go bowling or play bingo, and greatly enjoy having visitors over to their house. Staff provides support with functional life skills, behavior management, social integration, preparing meals, housekeeping, money management skills, medication administration monitoring, personal cares, shopping and community resources.
Foster 10 is located in South St. Cloud and is home to four individuals. They enjoy visitors, sports, arts and crafts, television, and community outings. Staff provide support with increasing independence with life skills, behavior management, social skills, community integration, medication administration monitoring, preparing meals, housekeeping, money management skills, shopping, and personal hygiene.
Foster 11 is located in Sauk Rapids and is home to four individuals. They enjoy pampering themselves, watching movies, arts and crafts, shopping, and staying active in the community. Staff provide support with increasing independence with life skills, behavior management, social skills, community integration, medication administration monitoring, preparing meals, housekeeping, money management skills, shopping, and personal hygiene.
Manor 1 is located in North St. Cloud and is home to four individuals. They enjoy sports, socializing, movies, video games, music, and being active in the community. Staff provide support with increasing independence with life skills, behavior management, social skills, community integration, medication administration monitoring, preparing meals, housekeeping, money management skills, shopping and personal hygiene.
Manor 2 is located in South St Cloud and is home to four individuals. They enjoy watching movies, grilling, animals, sports, music, and trips out into the community. Staff provides support with increasing independence with life skills, behavior management, social skills, community integration, medication administration, preparing meals, housekeeping, money management skills, shopping, and personal hygiene.
Prairie Hill is located in Southeast St. Cloud and is home to four individuals. They enjoy visiting, baking, arts & crafts, and community outings. Staff provides support with increasing independent living skills, medical needs, personal cares, medication monitoring, social skills, behavior management, preparing meals, shopping, housekeeping, money management, leisure and recreation at home and in the community.
Sterling Heights is located in South St. Cloud and is home to four individuals. They enjoy having visitors, community outings, television and movies, grilling, and enjoying their beautiful outdoor patio. Staff provides support with increasing independent living skills, medical needs, personal cares, medication monitoring, sensory needs, social skills, behavior management, preparing meals, shopping, housekeeping, money management, leisure and recreation.
Roosevelt II is located in Sartell and is home to one individual. They enjoy video games, television, music, and community outings. Staff provides support with increasing independence with life skills, behavior management, social skills, community integration, medication administration, meal prep, housekeeping, money management, and personal hygiene. A spacious Crisis Respite unit is located in the lower level of this home.
Christensen House is located in St. Joseph and is home to four individuals. They enjoy music, arts and crafts, going for walks and drives. Staff provides support with medical needs, personal cares, medication monitoring, sensory needs, social skills, behavior management, preparing meals, shopping, housekeeping, money management, leisure and recreation at home and in the community.
Oakland Heights is located in Sauk Rapids and is home to four individuals. They enjoy a large amount of in-home activities such as; movies, arts and crafts, video and board games, and community outings. Staff provides support with sensory needs, social skills, medication management, behavior management, preparing meals, housekeeping, money management, leisure and recreation at home and in the community.
Highview is located in Sauk Rapids and is home to four individuals. They enjoy sports, socializing, movies, video games, music, and being active in the community. Staff provide support with increasing independence with life skills, behavior management, social skills, community integration, medication administration monitoring, preparing meals, housekeeping, money management skills, shopping and personal hygiene.
