Supported Apartment Program

Opportunity Matters, Inc. Supported Apartment Services offers a person centered approach to integrated community living.
Opportunity Matters' Supported Apartment program is adapted to meet the unique needs of the individual by providing integration into their community through individualized outcomes. Consideration is given for the individuals preferences for location, apartment complex, amenities offered by the apartment complex, affordability, and ease of access to public transportation.

This program serves individuals on the DD/CADI/BI/CAC Waivers or private pay. The service is licensed by the Minnesota Department of Human Services under 245D Home and Community Based Services.

Opportunity Matters offers Living Skills, Individualized Home Supports, and 24 Hour Emergency Assistance services to individuals living in our supported apartment program.
