Pathway to Opportunities - Services



Adult Day Services: Our adult day services provide opportunities for individuals such as skill building, community and social opportunities, activities, health and exercise education.  Services are provided to those with cognitive and physical challenges (i.e. memory loss, developmental disabilities, traumatic brain injury, multiple sclerosis).  Services can be customized for individuals as needed.

Supportive Services:

(including but not limited to, In-home Family Services, Supported Living Services, and Independent Living Services)

Our supportive services are developed for individuals in their own living environment or in the community.  We serve individuals with a broad range of abilities and needs.  Services may include supervision for their daily living routines, assistance with accessing community resources, managing finances, grocery shopping, community education classes, assisting with seeking employment, social events, and becoming an active member of the community, etc.

These services help individuals exercise meaningful choice and control in their daily lives, and is designed to foster individuals' nurturing relationships, full membership in the community, and work toward their long-range personal goals.
