Corporate Foster Care Services: Opportunity Matters has 18 sites currently in operation, serving individuals on the DD/CADI/TBI/ CAC/EW/AC Waivers or private pay. Our sites are licensed by the Minnesota Department of Health, and have a program or service license from the Minnesota Department of Human Services under 245D Home and Community Based Services. Our dedicated staff members provide support to help individuals achieve their aspirations and goals.
Crisis Services: Crisis services provides immediate intervention for individuals experiencing either a medical, family, or behavioral crisis. In addition to defusing crisis situations, programs such as a behavior management or a medical plan may be provided that can be tailored to the individuals immediate needs, along with an assessment of needs, recommendations and referrals to appropriate agencies. Wrap around services may also be provided, so the assessments and tools recommended can be put in place when the individual leaves crisis, to ensure success in their previous or new placement. Crisis services consist of short term stays generally 30-90 days. Crisis services offer individual, secure units located within three of our foster homes. Transportation can be provided if needed.
To request crisis services, please contact Sam Pierskalla, Program Services Director of Opportunity Matters at (320) 240-1900 x 206.
Respite Services: These temporary services provide a break for the individual, caregivers, and families. Opportunity Matters can provide respite services in several of our corporate foster homes for as short as one day or for as long as six months. Respite stays can be reoccurring or scheduled in advance.
