This West St. Cloud home was opened in May of 1994 and is located next to Foster 2. A beautiful 4 bedroom patio home that is the residence to four very active and semi-independent women. This home encourages the use of independent skills, community resources, use of public transportation, as well as many community activities. Staff working here must be active, motivated, and able to multi-task. Staff must also be a motivator and willing to deal with repetitive behaviors.

Foster 3

 

This West St. Cloud home was opened in July of 1995. This 4 bedroom rambler has a beautiful deck and awning, a fenced in back yard and a pool table in the basement. The 4 men living in this home are supported in the use of independent skills, busing, accessing community resources, networking and community involvement. Staff must be able to be consistent, able to maintain boundaries, and able to handle potential for behaviors.

This East St. Cloud home was opened January of 2000. This 4 bedroom patio home with open floor plan allows for easy wheelchair access and supervision. The individuals living in this home are retired and do need assistance with personal cares and medical needs. Successful staff need patience, empathy, able to provide physical cares and maintain boundaries.

Moderate- profound functioning, need for personal care assistance, verbal/nonverbal, high frequency of mild to moderate challenging behaviors, high medical needs.

Foster 5

 

This Sauk Rapids patio home was originally opened in October of 1999 and rebuilt in 2007 to accommodate the needs of the individuals living there. With its large fenced in backyard these young ladies have plenty of room for fun outdoor activities! These 4 young adults are very active and spend a lot of time working on functional life skills, behavior management and social integration.

This Sauk Rapids bi-level home was opened in July of 2000. This high paced exciting home keeps everyone on the move. The young gentleman living here are very active and stay on the move! Individuals living here continue to work on life skills, behavior management, community integration and socialization.

This 4 bedroom Sauk Rapids patio home was opened in November of 2000. With a fenced in back yard the individuals living here are able to relax with privacy. The gentlemen living here live a relaxed life of retirement. They need assistance with personal cares and medical needs. Successful staff need patience, empathy, and able to provide physical cares.

This 4 bedroom patio home in St. Joe was opened in January of 2002 and is the home to very active individuals. The goal here is independence! Many of these individuals are working on life skills to increase independence both at home and in the community. They work on community integration and employment skills. They are very motivated and ready to be on the go.

This 4 bedroom modified 2 story patio home in St. Cloud was opened in February of 2002. In this relaxed setting with a structured routine the individuals living her continue to increase there independence, from community activities to busing. This site also houses a respite bed for individuals looking for part-time assistance.

This Sauk Rapids bi-level home was opened in August of 2003. The young women here are active, on the go and extremely creative! It is a fact paced home where everyone is on the move. Individuals living in this home work on skills to move them to semi-independent living. They work on cooking skills, the beginning basics of driving, employment skills and community integration.

Manor 1

 

This 2-story North St. Cloud home was opened in 1980 as an ICF/MR and was the organizations first home! Manor I was downsized to a 4 bed waiver and remodeled in the summer/fall of 2007. The young men living in this home are extremely athletic and active. It is a fast paced environment with high energy and physically active. Individuals in this home work on obtaining life skills such as cooking, working and busing to become as independent as possible.

Manor 2 - 1 Bed Open

This 6 bedroom SE St. Cloud 2-story home was our second home opened in May of 1984. The individuals living in this home continue to work on daily living skills such as cooking, cleaning and community activities. A few of these men have lived together for 15+ years and therefore promotes a sense of family in this home.

Christensen House

 

This St. Joseph home opened in October of 2006. This 4 bedroom patio home was designed to meet the specific needs of the consumers that reside there. The open floor plan allows for safe and comfortable mobility throughout the home. Staff remains busy during their shift preparing meals, bathing and interacting with the consumers. Successful staff must be able to multi-task and have lots of energy.

Significant amount of care needed for bathing, grooming, toileting, dressing and eating.

Activities are limited. They generally consist of walks, going out to eat and van rides.

This 4 bedroom patio home in St. Cloud was opened in February of 2004. Individuals living in this home require 24 hour assistance with community activities and personal cares and continue to work on life skills to increase independence. Relaxed setting where staff encourage independence. May required some assistance in supporting weight. Individuals here like to go for walks, movies and company activities.

This 4 bedroom rambler home in Sartell was opened in February of 2003. The individuals living here continue to work on daily living skills and behavior management. They enjoy participating in outdoor activities and games. They attend camp for several days during the summer and also enjoy walks frequently. It is a very structured home and some of the individuals enjoy helping with cleaning and cooking as part of their daily routine.
